High Five!

Now that we have received your medical records and insurance information for review, we will proceed to the final steps of our intake and screening process!

1) Video Call with your Case Manager: First, we will begin by having a video call with your Case Manager. During this call we will go over the surrogacy process, risks and responsibilities of being a surrogate, and compensation requests.In preparation for our call, you will be receiving a DocuSign email with several forms to review. You do not need to sign these yet. Simply review them.

We will go over each of them in detail during our call, so please jot down any questions you have prior to your chat. Once the call is over, we will ask that you review them one final time and then submit a signed copy to us.

2) Video Call with FMS Agency Director: During this call, you and your significant other will have the opportunity to meet with our Agency Director. This call gives you the opportunity to ask any questions you have, allows your partner to be part of the process, and helps us get to know you even better in preparation for matching. Many of the topics discussed will be your social, family and marital history, so that we can be sure all major topics have been covered before your psychological evaluation during medical screening. It’s also important for our Director to get to know you as much as possible so that she can identify the best possible match for you while she supports our Intended Parents. We will end the call with a brief “video tour” of your home, just so that we can confirm residency and safety. An in-person home visit may be required by your clinic in the future, so this is simply a casual snap-shot of your day-to-day life. No need to clean up on our account!

3) Background Checks: We will run background checks during this phase through a third party. You and any adults over the age of 18 will be required to have a background check. If you, your significant other, or any additional adults in the home have ANY history of criminal charges, arrests or legal disputes (even if expunged), NOW is the time to speak up. Please know that a history of criminal charges or arrests does not necessarily mean you will be disqualified from moving forward. We do however need to gather all important information since these questions will also come up down the road during your psychological assessment after matching.
